Photo Display Approval Form for photos of people displayed in booth; bring to fair and keep at booth; do NOT mail to IPFW

Student Information Form/Media Consent
The Student Info Form/Media Consent needs to be signed by the student's parent/guardian. This form will be at the student's display booth. If the student's parent/guardian will not be accompanying the student to the Fair to set-up, they should print and complete this form prior to the fair a nd send it with the student/accompanying adult on March 18.

Hoosier State Science & Engineering Fair Advancement
Elem/Junior/Senior Division (grade 4-12) will need to indicate their availability for the Hoosier State Science & Engineering Fair. Senior Division (grade 9-12) will also need to indicate their availability for the International Fair. This form will be at the display table or may be printed, completed, and brought to the Fair on March 18. It is submitted to the Display Inspector when display is checked.

Deadline March 3 at 5 p.m. to Register for Fair

Grades 6-12
Deadline March 3 at 5 p.m. to Register for Fair & submit online POST-review paperwork

Fair entry fee ($10 per project) due March 3
Remit
the project entry fee as a check or money order made payable to IPFW (cash cannot be accepted). Please indicate your school and the names of the registrants. Payment must be received by 5 PM on Friday, March 3 and may be mailed to:
