Request for a Home Contest
Use this form to request a Home Contest:
Directions on using the form
- Open the request for a Home contest form by clicking on the above link.
- Choose open.
- Place your cursor on the "Sport blank sport space" and click
- Type in your sport and tab to the next section
- Continue filling in the information on each section until you tab back to the top of the form
- Print a copy of the form
- Close the Adobe program
After you have a hard copy of the form complete the following steps:
- Facility Manager signs the form
- Athletic Administrator signs the request form
- Account Clerk makes a copy of the form for Sports Information Director and the Facility Manager
- Account Clerk creates a home contest contract
