ONLINE ELIGIBILITY FORMS
Junior/Senior Division (grades 6-12) Eligibility Procedures
for the Northeast Indiana Regional Science & Engineering Fair on March 20, 2010
CHANGES IN RULES FOR JUNIOR DIVISION (grades 6-8) FOR 2010 REGIONAL FAIR

Hazardous chemicals, devices and activities are prohibited by the Science Education Foundation of Indiana, Inc. (SEFI) for grade 6-8 projects.

Bacterial studies are permitted by Science Education Foundation of Indiana, Inc. (SEFI) under the condition it is conducted in a school setting using one of the approved bacterial cultures as long as the teacher is trained for BSL-1. BSL-2 levels are prohibited.
- The REQUIRED (Jr/Sr division) Pre-Experimentation Review Questionnaire must be filled out before you have access to the other eligibility forms.
- The Exemption form or Pre-Experimentation Review Coversheet will automatically be filled out if you finish all of the steps in the process immediately after filling out the questionnaire.
- An email confirmation will be sent to the teacher with the student’s ID Number. SAVE THIS EMAIL. The URL on the email will enable the teacher to access this student’s record as needed.

You must complete the Questionnaire to access the Online Eligibility Forms. Fill in student information and answer the 4 questions in the Pre-Experimentation Review Questionnaire.

Based on your answers, you will see a link to either the Exemption Form OR the Pre-Experimentation Review Cover Sheet.
CLICK the link(s*) to proceed
PRINT the appropriate form and follow directions.
*If the project requires pre-review, there is also a link to the required forms. Junior division uses Regional forms and Senior division uses ISEF forms.

| Step 3 |
Mail
- Pre-Experimentation Review paperwork: Mail with appropriate signatures to the Program Assistant at IPFW by January 27, 2010 deadline.
- Exempt projects: IF YOU ADVANCE TO THE REGIONAL FAIR, mail exemption form with appropriate signatures to the Program Assistant at IPFW by March 5, 2010 deadline.

If your project requires pre-experimentation review by committee, don't begin research until the project has been reviewed and approved.
