***NOTE***
Due to upcoming renovations, parking in the Gates Sports Center parking lot is extremely limited and we encourage fair participants to use the nearby parking garage.

Award Contact Info and Media Release Form
The Award Contact Info and Media Release form needs to be signed by the student's parent/guardian. This form will be at the student's display booth. If the student's parent/guardian will not be accompanying the student to the Fair to set-up, they should print and complete this form prior to the fair and send it with the student/accompanying adult on March 21.

Junior and Senior State Fair Availability Form
Junior Division (grade 6-8) will need to indicate their availability for the Hoosier State Fair on April 4. Senior Division (grade 9-12) will need to indicate their availability for the Hoosier State Fair and the International Fair May 10-15. This form will be at the display booth or may be printed, completed, and brought to the Fair on March 21. It is submitted to the Display Inspector when display is checked.
