Permissible Promotional Activity Request

The following forms are used to obtain permission to use a student athlete's name, picture, or appearance for charitable or educational purposes. Promotional Activity Form
Promotional Activity Signature Form
 Directions on using the forms Open both of the above forms and complete them electronically.
  1. Print the forms
  2. Close the Adobe program
After you have a hard copy of the forms complete the following steps
  1. Collect all required signatures from relevant Coaches and Student-Athletes
  2. Turn the form into the Compliance Coordinator
  3. Keep the copy for yourself
