Core Curriculum Summary

The following form is used to evaluate the initial eligibility of a prospect. Core Curriculum Summary
 Directions on using the form Open the Core Curriculum Summary form by clicking on the above link. Then choose open.
  1. Print the form
  2. Close the Adobe program
After you have a hard copy of the form complete the following steps
  1. Meet with the prospect
  2. Fill in the boxes
  3. Determine if the will be eligible
  4. If needed meet with the Compliance Officer for further clarification
  5. Keep the copy for yourself
